
Pet licenses will expire on June 30, 2025, for cats and dogs. The Paramus Health Department will be holding their annual free Rabies Vaccination Clinic on Thursday, May 1st, 2025, from 5pm to 7pm at the Paramus Life Safety Complex at 1 Carlough Drive, Paramus, NJ 07652.

Please bring the completed application with you to the Rabies Vaccination Program, or return/mail the application to the Paramus Health Department at 1 West Jockish Square, Paramus, NJ 07652, with a check or money order payable to the Paramus Health Department by June 30, 2025. Please mark envelope “Attention Dog/Cat License.” Also, if you have a cat or dog, you cannot put both animals on the same application. Instead, download an additional application, or pick up an application at the Health Department.
Certificate of rabies vaccination must be provided in accordance with New Jersey State Law and will be returned to you with your license. If you no longer have a pet, please submit the form below.
For a 1-year license, the vaccination must not expire before April 30, 2026. For a 3-year license, the vaccination must not expire before April 30, 2028. If it expires before these dates, you will need to have your pet revaccinated before a license can be obtained.
It is important to be aware that rabies is still present in wild animals in Bergen County. It is therefore vital that all cats and dogs be vaccinated and licensed.
BE SURE TO ENCLOSE:
1. A fee of $12.00 for pets that have been spayed or neutered and $15.00 for a non-neutered or non-spayed pet for a 1-year license.
2. A fee of $30.00 for pets that have been spayed or neutered and $40.00 for a non-neutered or non-spayed pet for a 3-year license.
3. Proof of rabies vaccination.
4. A check payable to the PARAMUS HEALTH DEPARTMENT
